Vendor Forms
Please complete and submit the attached vendor packet below to Julio De Hoyos, Procurement's Administrative Secretary at jdehoyos@laredoisd.org. If you have any questions, please contact Procurement's Front Desk at (956) 273-1080.
Please note: Submitting a vendor packet does not automatically make you an awarded vendor. If approved, your company will be set up on LISD's financial system. However, your company must still submit a bid for the products/services offered and undergo an official evaluation. If selected, a committee will recommend your company to the LISD Board of Trustees. LISD will distribute official award letters to all Board awarded vendors.
CIQ (Conflict of Interest Questionnaire) Form: Complete and submit this form as needed to Julio De Hoyos in Procurement at jdehoyos@laredoisd.org.
