HB 914 was approved by the Legislature and it is effective January 2006. In an effort to comply with this law, the District provides annual training to the Board of Trustees, the Superintendent and its employees that are involved in the monitoring and approval of contracts with vendors.

FORM CIS Local Government Conflict of Interest Statement [ This is the notice to the appropriate local government entity that the following local government officer has become aware of facts that require the officer to file this statement in accordance with chapter 176, Local Government Code ] Instructions for Completing FORM CIS FORM CIQ Conflict of Interest Questionnaire [ This questionnaire is filed in accordance with chapter 1176 of the Local Government Code by a person doing business with the governmental entity. ] By law this questionnaire must be filed with the records administrator of the government not later than the 7th business day after the date the person becomes aware of facts that require the statement to be filed. See section 176.006 Local Government Code. A person commits an offense if the person violates Section 176.006, Local Government Code. An offense under this section is a Class C misdemeanor.
